What it does

Careteam is a virtual care collaboration platform that works as a smart overlay across digital silos generating personalised plan of action for each patient and that everyone involved - from health teams to the patient and their family is on the same page. Sold to healthcare organisation as a SaaS license, it enables them to offer coordination and navigation for patients with complex, chronic diseases across different health conditions and contexts (hospital, clinic, community and home), reducing the fragmentation of care.  Careteam has been deployed by specialty programs within clinics and regions, as well as across ecosystems spanning hospitals, clinics and community organisations. We also support the current efforts to respond to the COVID-19 direct need to manage patients with suspected or confirmed illness at home as well as the indirect need to deliver care virtually in primary and specialty care settings.
